Dr Hitesh Patel is Consultant – Pain Management & Palliative Care he recieved his M.D. from B.J. Medical College Ahmedabad, Asia’s largest hospital, Civil Hospital, Ahmedabad. He earned his F.I.P.M. (Fellow In Pain Management) at Daradia Pain Hospital in Kolkata, which is affiliated with the Aesculap Academy in Germany. He received his C.C.P.P.M. (Certificate Course in Pain and Palliative Medicine) from Gujarat Cancer Research Institute Ahmedabad, the state’s largest cancer hospital.

Facet joints are lined with smooth cartilage and are lubricated with synovial fluid. Healthy joints are able to glide effortlessly as the spine performs movements, such as bending, twisting, and turning

  • The primary cause of facet arthropathy or spinal osteoarthritis is spinal degeneration which typically occurs in later life (disease of aging). 
  • Lumber segments tend to experience degenerative changes more frequently than other areas of the spine.
  • Over time, however, the cartilage can dehydrate and the synovial fluid can dry up.
  • Years of normal wear and tear on the facet joints lead to cartilaginous errosion, which can expose raw bone.



  • History and Clinical features
  • Medical imaging: X-rays, CAT scans, and Magnetic Resonance Imaging (MRI) may be used to exclude other abnormalities that may help in the diagnosis of facet arthropathy. 
  • Diagnostic injection. LA and dye are injected. If the facet joint is injected and pain relief is the result, that serves to confirm the diagnosis of facet arthropathy.
  • Low back pain  is the most frequent (Pain is generally a deep, dull ache) 
  • The pain is typically worse following sleep or rest morning stiffness.

Aggravated by:

  • The extension (Arching backward)
  • Sitting to standing
  • Rotational movement

Relieved by:

  • Flexion
  • Supine position


  • Physiotherapy, pharmacological    
  • Diagnostic facet joint injection under live x-ray guidance
  • Radiofrequency ablation
